Consent Form Agreement

A consent form will be emailed to you prior to your session once your session time is confirmed and payment is received. This consent form must be completed, signed, and returned to hello.innersoulpath@gmail.com at least 24 hours' before your scheduled session. If the completed form is not received by this deadline, your session will be rescheduled. Only one rescheduled session is permitted before your session cost is forfeited.
